The tool is a little hit-or-miss on some shapes.

The other one that sometimes does the trick is the knife tool, either the built-in one or the one from supercolormilk.com depending on whether or not you are working with a closed shape. Use the knife tool to cut a round opening from the main cylinder using the smaller cylinder as a knife. Don't remove the surface. Delete the smaller cylinder. Highlight the new surface you just made, and extrude to form shape projecting from the main cylinder.

In a pinch, you can also use Insert Vertex and Slice Surface to cut a hole by hand.
